Description of
call "Support to Member States and stakeholders to address the chronic disease challenge" | The objective of this action is to identify and address key elements with a potential to reduce the burden of major chronic diseases and increase the sustainability of health systems. It should provide new evidence on economic and efficient means to achieve better health outcomes and to alleviate the burden on health and social systems if implemented at an appropriate scale in Member States. Proposed initiatives should generate impact when implemented, be transferable to other settings and elaborate incentives for implementation. They should have the potential to become relevant elements of national chronic disease strategies and add up to an integrated response to chronic diseases. Initiatives should help to reduce avoidable costs and burden for patients and health care systems.

Applicant details | Grant applications are eligible if submitted by legal persons. More specifically, the applicants must be legally established organisations, public authorities, public sector bodies, in particular research and health institutions, universities and higher education establishments. The application shall state the legal status of the applicant. |

Project Partner Details | Applicants participating in a project proposal have to be different legal entities (i.e. independent from each other) from at least 3 countries participating in the Health Programme. Proposals which involve fewer applicants will be rejected. |

Financial details | INDICATIVE AMOUNT Support
to Member States and stakeholders to address the chronic disease challenge 2 500 000 €. Grants foreseen: one or more. The EU grant is limited to a maximum co-funding rate of 60% of eligible costs. In case of exceptional utility up to 80% can be granted. |
