EIT Health is offering between €300,000 and €500,000 in funding to support high-potential European start-ups developing innovative solutions in biotech, medtech, digital health, AI, and biomarkers or diagnostics. The programme is designed to help companies advance product development, strengthen commercialisation strategies, and prepare for their next stage of fundraising while addressing unmet clinical needs across Europe.
Expected effects and impacts
This call aims to strengthen integration between the research and business sides of the knowledge triangle by accelerating healthcare innovations borne out of research to support commercial and business growth of small and micro enterprises in European markets. All funded projects under this Call are expected to generate the following outcomes and long-term impact.
- Commercial acceleration, including attracting additional funding, closing future venture capital investments, and growing revenue (where applicable).
- Developing and accelerating new health innovations with high potential to positively impact the lives of the citizens of Europe, as well as to generate financial returns.
- Protection of new innovations and micro, small, and medium enterprises (SME´s) via intellectual property (IP) strategy.
Proposals must clearly and credibly articulate the path to achieving such impact, which may be reported up to three years after the end of the EIT Health funding period. Please refer to sections 2.3.3 on Key Performance Indicators (KPIs) and 7.2 on Monitoring for more information on the contractual obligations of projects to report achieved impacts during and after the project funding period.
Extension or continuation of projects will be tied to the conditions described in section 3.10 Fast Track Mechanism and Additional Project Funding.

Expected results
The Transformative Health Instrument (THI) call invites micro, small, and medium enterprises (SMEs) to submit applications for funding to support the development of scalable health technologies in Biotech, Medtech, Digital Health & AI, Biomarker / Diagnostics or a combination of these. Applicants should address unmet needs in Europe with novel and commercially viable approaches, scientific excellence, and strong internal capabilities.
Depending on the focus of the company, applicants’ solutions must be at the following innovation maturity levels (IML) at the time of application:
- IML ≥ 5 for Biotech (Safety/Toxicity study stage).
- IML≥ 6 in Medtech (Initial clinical PoC).
- IML ≥ 7 for Digital (Market validation).
- IML ≥ 7 for Biomarker Diagnostics.
The following types of innovations are out of scope:
- A non-medical device or application.
- A product primarily designed for wellness or lifestyle purposes that can be used without medical oversight.
- Companies that do not target the European market.
The funded innovation must be premised on a viable existing or future business model and a legally registered, for-profit entity with the proper capabilities to develop, market and scale the solution.

For this call, a ‘legal entity’ means a legal entity created and recognised as such under national law, EU law or international law, which has legal personality and which may, acting in its own name, exercise rights and be subject to obligations, or an entity without legal personality.
In accordance with the Horizon Europe Work Programme 2025 6, to be eligible for funding, the applicant must be established in one of the eligible countries, specifically:
- the Member States of the European Union, including their outermost regions;
- the Overseas Countries and Territories (OCTs) linked to the Member States;
- countries associated to Horizon Europe;
- certain low- and middle-income countries.
Individuals cannot apply for funding under this call.
Companies must meet the below conditions to be eligible for funding. Only companies satisfying all the below criteria will move to the evaluation stage. The following requirements and definitions shall apply to all applicants:
- The entity is a legally registered for-profit micro, small, or medium enterprise according to the European Commission SME definition.
- The entity must have at least 4 full-time equivalent (FTE), permanent employees who are compensated on a salaried basis, and are contracted to spend at least 50% of their total working time for the entity at the time of proposal submission. Staff hired as consultants or freelancers are not counted as permanent or salaried staff. The employment status of staff must be demonstrable through payroll records and employment contracts with the company upon request. This condition does not apply to the company Chief Executive Officer (CEO).
- The entity must have a CEO engaged on a full-time basis at the time of proposal submission. The CEO may be employed under an employment contract or engaged through an equivalent full-time arrangement (e.g. director mandate, management services agreement, founder-director role, or secondment), provided that the CEO is fully dedicated to the entity’s operations. Evidence of such engagement (e.g. employment contract, service agreement, appointment documentation or equivalent) must be available upon request.
- The entity cannot have publicly traded shares or be in the process of an Initial Public Offering (IPO).
- The assessment of the entity should be done at the parent (holding) company level, if the governance structure of the SME involves a holding structure.
- The company must have successfully closed an equity funding round (Qualified Investment Round) in the last 36 months by the application deadline, where the round meets the following conditions:
- A minimum total investment of 2,000,000 EUR within a 36-month period, with at least one new investor contributing.
- At least one of the new investors in the Qualified Investment Round must be a qualified, privately managed, profit-oriented, professional investor investing from a fund backed by Limited Partner (LP) commitments, including but not limited to independent European venture capital firms, firms managing alternative investment funds, and those investing private capital in early‑ and growth‑stage technology companies.
- The Qualified Investment Round must be a priced round with a set valuation. Funding rounds via a convertible note or Simple Agreement for Future Equity (SAFE) do not qualify.

The THI is intended to support private companies, specifically Micro, Small and Medium-sized enterprises (SMEs), with at least 4 full-time equivalent employees. Eligible companies must have closed a minimum total equity investment of 2,000,000 EUR within the previous 36-months, with at least one new investor contributing. Funded projects must be at the Innovation Maturity Levels (IML) listed below, depending on their industry focus:
- IML ≥ 5 for Biotech (Safety/Toxicity study stage).
- IML≥ 6 in Medtech (Initial clinical PoC)
- IML ≥ 7 for Digital (Market validation)
- IML ≥ 7 for Biomarker / Diagnostics
